Production Form

Welcome to Pact's online Production Information Form service. If you are producing a film or a TV programme that uses actors or musicians, then you will need to complete the following form online. The form provides Pact with data about your production's performers and crew, and helps us ensure that our services to you are as effective and useful as possible.

Completing this form does two things:

• If you are using a Pact Equity Agreement, it will tell us whether you are intending to lodge funds in escrow or if you will be providing a financial guarantee (see Appendix CCA of the Pact Equity TV Agreement or Clause F39 of the PACT/Equity Cinema Agreement for more details on this);

• On receipt of a form and on receipt of either a copy letter of financial guarantee or a notice that monies have been lodged in escrow, we will send you Artist Contracts with an authorisation number for you to use on your production;
